Friends of Parkwood Surgery

Membership

All patients of Parkwood Drive Surgery will be automatically enrolled in The Friends of Parkwood Surgery (FPS) – your patient group.

Being a member of FPS enables us to contact you to share information, seek your input and keep you up to date with FPS initiatives; and enables you to engage with the FPS committee.

All patient information and contact details are held and managed by Parkwood Surgery: no data is shared with the FPS committee, and all communications to members will be issued by Parkwood Surgery on our behalf.

For Parkwood Surgery to notify you about FPS Communications, please ensure that they have your mobile phone number on record.

To add your mobile phone number to your record, you can submit an online request via the Parkwood Surgery website.

Membership of FPS will cease if you leave Parkwood Surgery.

If you wish to opt out of being a member of FPS, please contact Parkwood Surgery.

If you are a patient of Parkwood Surgery, do not have a mobile phone but would like to receive FPS communications, please use our Contact Us form to request emails directly from FPS. We will require your email address and confirmation that you are a patient of Parkwood Surgery, Hemel Hempstead. You can opt out of email communications at any time by using our Contact Us form.

Membership is free.

The committee does raise funds to cover its basic administrative costs, to fund speakers for health talks and to contribute to the purchase of additional items for the surgery that they may not otherwise be able to obtain.

Donations are completely voluntary; if you would like to make a donation to FPS, you can:-
